Daxxify vs. Botox: Which Neurotoxin Lasts Longer?

Botox® (onabotulinumtoxinA) and Daxxify™ (daxibotulinumtoxinA-lanm) are both neuromodulators for movement-related lines. Daxxify’s formulation and kinetics have positioned it as a strong option for patients who value faster perceived onset and longer maintenance intervals. Remember: units are product-specific and not interchangeable - dose to the product label and your training, not by 1:1 “unit matching.”
Natural-Looking Results (Injector Framing)
For many patients, “natural” means softening movement without freezing expression. In our experience, Daxxify can deliver a smooth, soft appearance when:
- you dose to indication rather than unit-converting from other toxins,
- you map injection points to actual dynamic lines (not just habit), and
- you pair it with patient education on the settle window (1 - 2 weeks) and maintenance cadence.
Position this clearly in consults: Daxxify aims to enhance skin’s visual quality and refine expression lines while maintaining a patient’s natural look - especially valuable for patients on camera, in sales, or those who’ve been “over-treated” elsewhere.
Choosing Daxxify: Considerations for Consult & Candidacy
As with any neuromodulator, assess:
- Goals & lifestyle: event timelines, desired visit frequency, tolerance for gradual onset vs. rapid polish.
- Dynamic pattern & muscle strength: tailor dose and spread to forehead, glabella, and canthus behavior.
- History: prior toxin response, duration, and any variability in effect over time.
- Adjacencies: whether they’ll benefit from a movement + shape + skin plan (e.g., tox + filler + a skin-quality tool such as Skinvive, plus skincare/SPF).
Set expectations up front: onset can be felt quickly for some, but the best readout is usually at 1 - 2 weeks; durability depends on individual biology and treatment area.
Botox vs Daxxify: Our 4-Month Split-Face Series (Educational)
Design (December 2022 start): Led by Dr. Michael Lafkas, we ran a small, internal, split-face case series (n=7) comparing Daxxify to Botox over four months. This was an educational demonstration, not a randomized controlled trial. We captured procedure and progress videos at multiple time points.
What we saw (high-level):
- 3-Month Signal: Daxxify looked better for longer in most participants. On the Botox side, movement returned earlier.
- Region Differences: Daxxify stood out in the glabella (we observed ~50 - 80% longer-lasting softening vs. Botox in this area) and performed well around the lateral canthus; forehead outcomes were closer to parity.
- Participant Preference: Most preferred the Daxxify side aesthetically by Month 3.
- Ethics/Comfort: To avoid prolonged asymmetry, we re-treated the Botox side at ~3 months and concluded the series at Month 4.
Cost notes (practice ops): While some clinics price Daxxify lower per unit, total per-treatment cost may be higherdepending on dose - again, units are not interchangeable across brands. Many patients still favored Daxxify given fewer visits and the perceived longevity.
A professional’s perspective: In 18 years of practice, Dr. Lafkas has observed that some patients perceive diminishing benefit from their historical toxin choice. For those patients, Daxxify offers a credible alternative - and enthusiasm has been high.

How to Position Each in Your Menu
- Botox®: Gold-standard familiarity, robust data history, wide patient awareness, flexible dosing strategies.
- Daxxify™: Strong fit for patients who prioritize longer intervals and quick polish - use consult scripts that emphasize natural expression and a predictable settle window.
Program idea: Build two clear pathways on your service menu:
- Classic Maintenance: 3–4 neuromodulator visits/year (Botox).
- Extended Maintenance: ~2 neuromodulator visits/year (Daxxify), personalized to response.
Bundle either pathway with skin-quality and skincare touchpoints for holistic results.
FAQs
Which lasts longer in practice?
In our small split-face series, Daxxify maintained effect longer - especially in the glabella - while forehead differences were smaller. Individual biology, area, and dose matter. Your patients’ mileage will vary.
How fast is the onset?
Some patients feel/see change within a couple of days; set expectations that best read is at 1 - 2 weeks for either product.
Can I convert units 1:1?
No. Units are not interchangeable across neuromodulators. Dose to the label, your training, and the clinical presentation.
Is the result “frozen”?
Not when mapped and dosed correctly. With appropriate planning, both products can soften movement while preserving expression; many injectors report particularly soft, natural finishes with Daxxify.
